That form is old, OLD. I vaguely recall it. I think it's 
from the '50's/'60's era IIRC. You should be able to get
an answer from the FCC's Office of Secretary. They handle
all the library materials there.

I'm thinking it might be an auxiliary application license 
form that was paired with Form 351 and might have been used 
to apply for license after STL or RPU construction years ago,
but I'm not sure.
