We lived by the BSP (Bell System Practices) when I was in operations.
 
Working nights in the Central Office gave us LOTs of time to be studying  the 
BSP's as we routined the gear which in the early 1960's was mostly vacuum  
tube.
 
For some more good reading see Bernard Mager's, "75 Yeas of Western  Electric 
Vacuum tubes."  Quite a book.
