[BC] Origin of acronym QKT

www.bellsystemmemorial.com has a lot of BSP's relating to basic telephone
practices.  I have a number of BSP's relating to broadcasting that I keep 
meaning to clean up onto a CD-ROM and send to Barry for oldradio.  (I haven't
forgotten, Barry, honest!)
