In lots of recent cases, the worms that exploit security weaknesses
would have no effect whatsoever on a Windows machine that has up-to-date
patches.  In addition to keeping your antivirus software, whichever
flavor you like, current on its definition set, you also need to make
sure that you have installed every Windows patch labeled as critical. 
Usually, Windows security patches are issued at the beginning of each
month, but occasionally they'll issue one almost immediately upon
discovery.  Always make sure you have Automatic Updates set up to
download all critical updates, and never ignore the Windows icon in your
tray.

Lots of the recent security-hole-exploit worms have passed our office
by, because we use a Windows Server running Software Update Services. 
It pushed the fixes to all our workstations well before any of the worms
were "in the wild."
