What you are describing here sounds like poor connection in the fuse  
holders! Is it possible to insert an in-line current meter in series with the  fuse 
to determine the actual operating current?
 
If you have a high resistance connection in the fuse holders, corrosion  etc. 
this may be a clue.
 
It is curious that the manufacturer places two fuses of differing current  
capacity in series. 
This may be a "safety fusing arrangement," in that for low current shorts,  
only the low amperage fuse goes. If it is a cataclysmic failure one of both  
fuses will go.
 
In the telephone business we used low current indicator fuses "across," the  
higher ampacity fuses to indicate when for example, a cartridge type fuse 
blew.  The indicator fuses were tied into the central office's alarm system and 
would  alert the supervising office of a fuse failure.
